Development and Validation of an Advanced Competency Framework for Pediatric Hemato-Oncology Nurses in the Italian Context

Abstract

BACKGROUND: Pediatric hemato-oncology is a highly complex clinical field characterized by advanced clinical, relational, and ethical care needs. In Italy, there is currently no formal recognition or national Core Curriculum for the Pediatric Hemato-Oncology Nurse (PHON), resulting in educational and organizational variability.

AIMS: To define, through a structured consensus process, a set of advanced competencies aimed at developing a national Core Curriculum and Job Description for PHONs within the Italian healthcare context.

METHODS: This methodological study was conducted in two phases: an initial review of national and international literature to identify preliminary competencies, followed by a two-round electronic Delphi process involving nurse representatives from AIEOP centers. Competencies were evaluated for clarity and relevance using a 5-point Likert scale (0–4). Consensus regarding the level of expertise required for professional autonomy (novice, intermediate, expert) was defined as ≥75% agreement.

RESULTS: Nineteen nurses participated (mean age 45 years; mean 18 years of experience in pediatric hemato-oncology). Following the first round, textual and structural revisions were implemented. After the second round, a Core Curriculum comprising 126 competencies organized into nine macro-areas was established. Professional autonomy was expected within the first year for 58 competencies, after 1–3 years for 46, and after more than three years for 22 competencies.

CONCLUSIONS: The Delphi process enabled the development of a shared national competency framework, providing a methodologically robust foundation for the recognition and advancement of the Pediatric Hemato-Oncology Nurse role in Italy.
